Position Description & Requirements

Action Against Hunger is recruiting a Nutrition and Health Officer to support the implementation, monitoring, supervision and quality improvement of nutrition and primary-healthcare services in Sokoto State.

The position is a full-time, mid-level role reporting to the Deputy Programme Manager for Health and Nutrition. The proposed start date is August 2026.

The successful candidate will work with health facilities, government health personnel, community health workers, Nutrition and Health Assistants and other programme teams.

Key Responsibilities

Programme Monitoring and Data Management

  • Update and maintain the Health and Nutrition 5W/4W mapping tool for the operational Local Government Area.
  • Monitor programme screening data relating to acute malnutrition.
  • Collect health-facility disease-surveillance information and update morbidity-monitoring tools.
  • Participate with the Monitoring and Evaluation team in supervising nutrition and health assessments.
  • Maintain and regularly update databases for health facilities, community-support groups, health workers and community volunteers.
  • Prepare consolidated tracking tools for supportive supervision and on-the-job coaching.
  • Update Local Government Area work plans and individual staff work plans.
  • Complete monthly health-evolution reports and submit them for consolidation.

Health-Worker Capacity Building

  • Conduct regular assessments of health workers’ capacity to deliver CMAM, IMCI, reproductive-health, vaccination and other primary-healthcare services.
  • Identify training and coaching needs among health workers.
  • Support the organisation of technical training, including LMIS and HMIS training.
  • Provide on-the-job coaching and supportive supervision.
  • Ensure that Nutrition and Health Assistants use approved quality-assessment and supervision checklists.
  • Maintain proper records of coaching sessions, training, workshops and meetings.

Quality of Healthcare Services

  • Ensure that children and other patients visiting supported health facilities are examined and treated according to national health and nutrition protocols.
  • Monitor the quality of CMAM, IMCI, antenatal, postnatal, reproductive-health and vaccination services.
  • Ensure correct completion and storage of CMAM and IMCI registers, growth-monitoring cards, antenatal and postnatal cards and delivery registers.
  • Identify service-delivery gaps and recommend corrective action.
  • Ensure referrals are made and tracked for high-risk pregnancies, severe acute malnutrition with medical complications and other conditions requiring higher-level care.

Supplies, Logistics and Facility Support

  • Monitor the availability of essential medicines, nutrition commodities, medical supplies and data-collection tools.
  • Identify supply shortages early and report them to the line manager.
  • Prepare monthly supply requests.
  • Collect and consolidate consumption reports from supported health facilities.
  • Promote the rational use of medical equipment, supplies and stationery.
  • Identify WASH gaps within supported health facilities and report them for follow-up.
  • Review programme financial, logistical and human-resource needs monthly and quarterly.

Coordination and Government Engagement

  • Support Local Government Area coordination meetings.
  • Facilitate quarterly review meetings and workshops with government health managers at state and local-government levels.
  • Support government health workers in preparing DHIS2 and Integrated Disease Surveillance and Response reports.
  • Maintain regular communication and coordination with programme management.

Supervision and Team Management

  • Prepare monthly monitoring and supervision plans.
  • Supervise Nutrition and Health Assistants and other assigned personnel.
  • Monitor staff attendance, validate monthly attendance records and submit them to Human Resources.
  • Follow up on staff performance, work plans, leave, appraisals and capacity-development needs.
  • Ensure that staff receive orientation on Action Against Hunger’s charter, values, safeguarding requirements and code of conduct.
  • Provide guidance to team members on implementation challenges and corrective actions.

Planning and Reporting

  • Prepare monthly individual action plans based on the programme work plan.
  • Report significant contextual changes affecting nutrition and health operations.
  • Report progress made through training, supportive supervision and on-the-job coaching.
  • Document service-quality findings, operational challenges and proposed solutions.
  • Submit accurate and timely reports to programme management.

Qualifications and Experience

  • Bachelor’s degree in Nursing, Midwifery, Public Health or another relevant health qualification.
  • Experience in public health and Community Management of Acute Malnutrition programming.
  • Minimum of three years’ experience implementing primary-healthcare programmes.
  • Strong report-writing skills.
  • Proficiency in Microsoft Word, Excel and PowerPoint.
  • Fluency in spoken and written English and Hausa.
  • Good communication and interpersonal skills.
  • Experience managing teams working on similar programmes.
  • Ability to observe service delivery and provide constructive feedback.
  • Ability to manage staff in accordance with organisational policies and the local cultural context.
